1. DOCUMENTS NECESSARY TO OBTAIN A NEW PENSIONERS CGHS CARD.

 

            i.   Please bring Pension payment order in original if the pension has been finalized.                        Pension payment order copy duly attested to be enclosed/ Last pay drawn certificate in original to be enclosed.

      ii.   3 latest joint photograph with the eligible family members attested by gazetted officer.

      iii. Surrender certificate of CGHS identity card issued while in service.

      iv. If not availed CGHS facilities, please get the certificate from the department 'Not availed CGHS facilities while in service'.

 

2.    PROCEDURE TO BE FOLLOWED FOR USG ABDOMEN AND PELVIS.

          After taking a appointment over telephone/in person from the attached polyclinic patient has to take 2 tab of antacid and 2 tablets of laxatives (dulcolax/cremalx) on the previous night of the appointment day. Preferably on empty stomach and full bladder with lot of drinking water.

 

3.    PROCEDURE TO BE FOLLOWED FOR X-RAY KUB AND LS SPINE.

          Patient has to take 2 tabs of antacid and 2 tablets of laxatives (dulcolax/cremalx) on the previous night and on empty stomach.

 

4.    DOCUMENTS NEEDED FOR ISSUING PERMISSION LETTER TO PRIVATE RECOGNISED HOSPITALS/DIAGNOSTIC CENTRE.

      a. A photocopy  of the  valid cghs card

      b. A request letter from the beneficiary indicating the choice of hospital.

      c. A photocopy of the prescription from the CGHS/Government specialist.

 

5. What is the time period for submission of medical reimbursement claim bills?

         The bills must be submitted within 3 months from the date of discharge/last consultation/investigation.
